Information from your campaign is important to
the United Way of Will County. United Way has provided a Report
Form with your campaign materials. The following information
should prove helpful as you wrap up your campaign.

Complete the
Report Form,
Side 2
retaining a copy for your files. Please provide all
the information requested on this form, including the number
of employees, number of contributors, and percent
participation. Please do this prior to submitting results
to your corporate headquarters, if applicable.

Please include copies of all
pledge forms for contributors. You should retain the
originals for payroll deduction authorization.

Enclose all cash and checks
and itemize on the cash/direct bill lines. Do not
separate cash and checks from the corresponding pledge
forms. Please make sure that the total amount pledged,
cash/checks, balance due, and payroll deduction pledges,
balances with the pledge forms and figures shown on the
Campaign Report Form.

Mail to the United Way of
Will County office or call to arrange a pick up

Scorecard
A
Campaign Scorecard
will be included with your Campaign materials. The
Scorecard provides the United Way office with information we
need to recognize your campaign successes and allows you the
opportunity to evaluate your campaign efforts. Points are
assigned for per capita giving, percentage of participation and
a variety of campaign communications and achievements. Please be
sure to complete this form in order to be acknowledged in the
United Way of Will County's Campaign Celebration program book in
February. |
