If you want to designate your gift to one of our Agency Partners, or to another United Way, you must print and submit the designation form with your pledge card.
The Designation Form is required annually, for those who designate their gift. Rollovers and Designations don't mix. If you have your gift on automatic rollover through your employer, and you want your gift designated, you must still complete the Designation Form annually.
- The Designation Form lists all Agencies in one spot, thus making your Agency Designation selection quick and convenient!
- If choosing to send your gift to another United Way, the form requests specific contact information, to help us make sure we are sending your gift to the correct United Way! Believe it or not, Google doesn't always provide us with the correct information!
- In some instances, when donating to another United Way, donors have only written their county name. We do need complete information for the United Way, as some counties have multiple United Way's covering their community. Example: DuPage County could either be Fox Valley United Way or DuPage/West Cook United Way.
Ultimately, the Designation Form increases our efficiency in processing gifts, providing all necessary information in one, convenient location.
